  • Procedures for scheduling appointments


    • Please email the following: Home address, Date of birth, Image of insurance card (front and back)
    • Please take a moment to call your insurance provider or check their website for information about your particular plan's In-Network and Out-of-Network Chiropractic coverage: deductibles, co-pays, co-insurance and visit limits.  Attention Blue Cross/Anthem Patients:  Ask if your claim will be processed through Blue cross or American Specialty Health.  Obtain address here we should submit your electronic claims.
    • Once we receive the requested information, we will contact your insurance and verify your benefits.
    • We will call you the same day to discuss your insurance benefits and schedule an appointment.
    • Appointments can only be reserved on the phone with a credit card.  Your credit card will only be charged if you do not show up to your appointment.

    Our cancelation policy requires 48 hour NOT 24 hour notice.  

    Cancellation / Rescheduling Policy: We understand there are times you must change or cancel your appointments and we ask for the courtesy of 48- hour notice for Tuesday, Wednesday, Thursday and Friday appointments. For Monday appointment's, we ask for notification by the preceding Thursday.  Your timely notice allows us the opportunity to offer your appointment to another patient.  Appointments canceled or rescheduled with less than 48- hour notice are subject to $65 fee.
